Self-Certification (SC2 Form) 

If you’ve been unwell for more than four consecutive days but less than seven, you can self-certify your illness using an SC2 form. This form is available from your employer or the HMRC website https://www.gov.uk/guidance/ask-your-employer-for-statutory-sick-pay

 


 

More than Seven Days Off Sick

If your sickness absence exceeds seven days, your employer will usually require a Fit Note (also called a Statement of Fitness for Work MED3 form) from your GP or hospital doctor.

Please note: We do not consider issuing or renewing Fit Notes as a same-day urgent medical need. For continuity of care, we recommend discussing this with your named GP.

We understand that some patients worry about delayed pay if a Fit Note isn’t provided promptly. However, employers cannot legally withhold pay without proper cause or consent. If needed, show proof through the NHS App that you’ve submitted a Fit Note request via accuRx. You should not be penalised due to delays in NHS processes.

Fit Notes can be backdated. Depending on your condition, a face-to-face GP assessment may be required.

 


 

Hospital Admissions

If you’ve been admitted to hospital, a Fit Note can be issued by the hospital for the duration of your stay. After discharge, your GP can provide a follow-up Fit Note based on the hospital’s discharge summary.

Private Medical Certificates

If your employer requires additional documentation beyond the standard Fit Note, your doctor may issue a private letter. Please note that this service will incur a charge. Contact our Reception Desk for more details.
